Schedule your first unhurried and in-person visit with Dr. Monthie at our office.

Dr. Monthie will request and review any relevant medical records from your previous doctors, specialists, and hospitalizations to prepare for your first visit.

Complete our intake forms online at your leisure.

Schedule a free virtual Meet & Greet

Arrive to your first visit at Heirloom and experience primary care how it should be - without any wait, copay, or stack of redundant forms.

Our Generations membership includes up to 2 adults, all dependent children under age 26 or who are still in school, and any other dependents under your care.

(P.S. We know families come in all shapes and sizes.

Health is cultivated over time, with intention. Make this annual investment in yourself to make it even easier to plant the seeds for a healthier year ahead.

There is a one-time enrollment fee of $150 per member (max $300 per family) to cover the time, care, and behind the scenes effort it takes to thoroughly gather and review your medical history so we can begin our care relationship on the same page.

There is no long-term contract, just a relationship built on mutual trust and respect. If you ever need to step away from your membership, we ask for 30 days' notice to wrap up and transition your care thoughtfully. 

Rejoining later is subject to availability and may include a $500 re-enrollment fee.

We celebrate those who have reached this milestone birthday by meeting their health needs with the dignity and attention they deserve.

Centenarian membership

Former patients are invited to join Heirloom with no enrollment fee (just a hug and warm welcome back!) if enrolled by December 31, 2025.
