We see patients with all types of insurance, but the way we interact with insurance is different - and that’s one of our biggest flexes!
Your membership covers all visits, communication, and care coordination without copays or surprise bills.
You can still use your insurance for things like labs, imaging, prescriptions, specialists, and hospital visits.
That said, we aim to keep you well and out of urgent cares, EDs, and hospitals with better access and more thoughtful care.

Frequently Asked Questions
Membership models of healthcare may be new or seem unnecessary to you, but we’re here to walk you through it! The short answer: DPC sets patients AND their doctors up for success.
Think of Heirloom like joining a gym, but for your health. Your membership gives you consistent, personalized support from a doctor who knows you well. Use it as much or as little as you need, but take comfort in knowing we'll always be here when you need us. (P.S. Our membership fee is less than that of many local boutique gyms).
Heirloom isn’t a nurse line giving generic advice or a random teledoc or non-physician provider who knows nothing about you. We’re your personal doctor who is just a call (or message, e-mail, virtual visit, or in person appointment) away for all of your health concerns, big and small. It’s like having a doctor in the family.
You’re not just a number here. Because Heirloom limits its panel size to ~250 patients (vs. the typical 2500+), you get:
- more time with your doctor,
- same or next day visits,
- better coordination of care,
- a relationship that grows with you and your family.

Heirloom delivers a concierge level of care without the concierge price tag.
We are a direct primary care (DPC) practice, a growing care model gaining traction across the country because of its patient/doctor relationship focus.
Unlike traditional concierge practices that charge high fees and bill insurance, DPC keeps it simple: one affordable monthly fee, no insurance billing, no hidden costs.
DPC is concierge-style care for everyday people like you and me!
Even with "good" insurance, many patients still face long waits, rushed visits, and disconnected care.
Simply put, we offer a level of unprecedented access and hospitality that no traditional doctor’s office can offer. Ask yourself:
> Do you have your doctor’s cell phone number?
> Can you be seen when you. need to - and by your actual doctor?
> Does your visits last longer than 10 minutes?
> Are your messages and calls returned quickly?
> Have you ever gotten a surprise medical bill or avoided care due to hassle or high cost?
> Are you bouncing between urgent cares, specialists, the emergency department, and various tests without any answers or help connecting the dots?
> Does your current primary care experience leave you feeling like just another number?
Sound familiar? Heirloom is different and was built with you in mind.

That's great! But even if you haven’t been diagnosed with any chronic medical conditions, it’s likely you still have things you want to improve about your health, like more energy, better sleep, or less stress.
At Heirloom, we take the time to explore it all - your habits, routines, and what's getting in the way of your best self - so we can be proactive, not just reactive.
If you’re healthy now, let’s keep it that way - on your own terms.
You're welcome at Heirloom! We can care for you regardless of insurance status.
That said, we recommend keeping insurance or a health-sharing plan for things like medications, labs, imaging, specialists, and emergencies.
Dr. Monthie is extremely dedicated to her patients and offers responsive, high touch care, usually replying within a few hours during business hours. If you need to be seen, we can almost always get you an appointment the same or next business day.
Unfortunately, no doctor can be available 24/7.
As a wife and mom of two young children, Dr. Monthie typically reserves nights and weekends with her family and responds to non-urgent messages when the office reopens.
Upon enrollment, we will review best practices for communication, including what to do if concerns come up after-hours and on weekends.
We think so. For less than a daily coffee run, you get direct access to a trusted physician and all of the comprehensive primary care services you need so you can focus on the things that really matter.
Today’s common high deductible plans often mean you're paying thousands out of pocket before your insurance even begins to help. For many families, it feels like they’re footing the bill and navigating care alone, all while still paying high monthly premiums.
In a traditional insurance-based primary care setting, a simple visit for a cold can cost more than $150 out of pocket ($25-50 copay + $100+ of surprise bills sent to you weeks to months later).
You might be surprised to learn that avoiding one ER visit or urgent care trip often offsets the entire year’s membership.
Many DPC patients also pair their DPC membership with a lower cost health sharing plan/ministry or a DPC-friendly insurance option to cover both everyday care and the what-ifs.
Missed diagnoses, hasty appointments, confusing bills, long waits, and doctors who don’t really know you have become the standard of "care." But here’s the truth: delayed care comes at a cost - in time, in money in mental load, and in long term health outcomes.
Can you afford NOT to have an Heirloom level of care? And what could you gain by investing in care that actually invests in you?
Many members use their HSA or FSA funds for their membership. In fact, the new Big Beautiful Bill classifies DPC membership fees as a qualified medical expense that permits use of HSA funds to cover up to $150/month per individual and $300/month per family beginning January 1, 2026.
We also recommend checking with your tax advisor to confirm eligibility.
Vaccines are one of the most powerful tools we have to prevent serious diseases and protect vulnerable individuals within our communities.
At Heirloom, we firmly believe in the safety and effectiveness of vaccines and recommend following the schedules set by the Centers for Disease Control and Prevention (CDC) and the American Academy of Pediatrics (AAP) which are based on extensive research, evidence, and expert opinion.
With that being said, we know every family is unique, and we respect your family's opinions and wishes. While many pediatric and family medicine practices are not able to accomodate families who delay or decline vaccines, we believe that all families deserve a trusted medical home, not exile. At Heirloom, we welcome open, respectful conversations and meet families where they are, field concerns and questions, and provide guidance so that parents feel heard and supported when making decisions about their family’s health.

You're welcome at Heirloom! We have formally opted out of Medicare which means:
1) We don't bill Medicare for our services,
2) You won't be reimbursed by Medicare for care at Heirloom,
3) You'll sign a private contract agreeing to pay out-of-pocket for your membership.
You can still use your Medicare benefits for everything else—labs, imaging, medications, specialists, and hospital care.
You're welcome at Heirloom! However, Heirloom does not bill Medicaid, so your membership would be entirely out-of-pocket and not eligible for reimbursement by Medicaid.
You can still use your Medicaid coverage for labs, medications, imaging, specialist care, and hospital visits outside of Heirloom.
Many patients choose to pair their Medicaid benefits with DPC because they value the consistent, personalized care they can’t get anywhere else.
